"Brain-Freeze" and "Ice Cream...BACKache"?

Ever get a "brain-freeze"..that intensely painful (but mercifully brief) "ice cream headache"* that some 30% of us experience when we eat or drink something cold too quickly?


*For the record, "ice cream headache" , "brain-freeze" or sphenopalatine ganglioneuralgia is defined as a "headache attributed to ingestion or inhalation of a cold stimulus", and medically it seems to be of zero interest, other than something about which to occasionally reassure worried patients, because even though it really, really hurts, ice cream headache is
1.self-limiting
2. not dangerous
3. nobody dies from it (directly)
4. avoidable in the first place
... therefore it's really of no importance, health-wise.

Brain-freeze is usually less than a minute in duration (often much less), but while it's going on, it can feel like Hell's actually freezing over, inside your head, with piercingly intense pain felt mostly in the frontal areas of the head. (Ow! Ow! Ow! Ow!! OW!!!)

Unofficially I'd guess that (here in the U.S.A.), the main trigger for "brain freeze" seems to be sucking a blended drink made mostly of ice through a straw, such as a "Slurpee", "Slush", or frozen Margarita; ice cream and especially ices and sherbets probably come in a close second. (My Dad, an intelligent man given to strong (if sometimes overly broad) opinions, avoided eating sherbet because he'd always experienced the ice cream headache from sherbet, but rarely from ice cream. Something about consistency I guess..)

Here's how it works: Ingesting the icy substance (especially through a straw), rapidly cools the hard palate causing vasoconstriction of the descending palatine arteries. The trouble seems to start when the area begins to warm back up quickly during the acts of breathing and speaking, which reverses the vascular action causing immediate and extreme vasodilation , which is interpreted by the nerves in the area as pain. (Sounds like a brief, mislocated migraine to me.)

The thing is, some of us feel our version of the "brain-freeze" between the shoulder blades instead of the head. I don't know what else to call it but an "ice cream backache". I'm not implying it's of medical significance, I just think it's interesting.

From childhood I experienced the typical "brain-freeze", but about 20 years ago (in my 30's) it suddenly moved out of my head and relocated in the middle of my back, deep between the shoulder blades, which I liked even less.

With the "brain-freeze", at least I could apply strong counter pressure with my hands, which was some comfort. But when ice cream backache strikes, all one can do is press back hard against a chair or a wall, or just squirm. (
And I know from experience that that "squirming" reaction is not only undignified but can tend to alarm onlookers, who may think you're having a heart attack or stroke or something. No, it's nothing that you'll get any sympathy for, because by the time you've explained what's happening to you, it's over and the onlookers are rolling their eyes..).

Anyway, I'd never heard of that between-the-shoulder-blades variation on the "brain-freeze" before I experienced it myself, and my neurologist had never heard of it either, so I assumed I was alone in this unpleasant, unimportant weirdness... story of my life, really, but that's another blog...


Luckily, in 1997 I found a bona fide medical article on the subject of "Ice Cream Headache", published in the "British Medical Journal" by Dr. Joseph Hulihan, Professor in the Department of Neurology at Temple University Health Sciences Center. http://www.bmj.com/cgi/content/full/314/7091/1364

Dr. Hulihan seems to be the world's only "I.C.H." (Ice Cream Headache) expert, although that's not his focus since it's probably not enough to build a career around. But he's the authority. And fortunately, Dr. Hulihan's artcle on "Ice Cream Headache" was followed by a Q & A board on the subject, so I posted a question asking whether anyone else had experienced (or even heard of) what I described and called "ice cream BACKache".

Well, it turns out I wasn't alone at all - several people wrote in that they'd experienced the same thing, "ice cream backache", and they all thought they were unique in this.


I wrote to Dr. H. about it, and he wrote back to say that he'd never heard of this variation on ice cream headache, and found it interesting. He surmised that it might have something to do with referred pain from esophageal spasm.

Since Dr. H. hadn't heard of it, I assume that ice cream backache is relatively rare, and as far as I've been able to tell, no one is studying (or has ever studied) it, so it's not well understood. But it does happen.

It's not an important issue, medically speaking, or even remotely dangerous, but personally I'm curious and would like to understand it. I'm hoping to eveventually collect some basic data about this.

I have always gotten ice cream backaches. I'm 24 years old tomorrow. I looked up and found this blog by googling just that... ice cream backache because wikipedia only had ice cream headaches.

I know this blog is old.. but I too get ice cream backaches... and it started in my twenties. Late teens and twenties also kicked in a lot of heartburn so I thought the esophagus spasm theory is interesting and maybe there is a realtion to those who get frequent heart burn also getting Ice cream backaches.

Aha! I love that I am not alone! I had the regular ice cream headache as a kid, but by the time I was a teen it was right between the shoulder blades. I have been told this 'wasn't possible' more than once. Vindication at last!

I get ice cream backache too and have always wondered about it. Mine is not between the shoulder blades, but much lower on my left side. I always tell people I'm getting ice cream kidneys.

I get this same thing! Not so much with ice cream since I don't eat it, but with ice. I started eating ice chips because I was missing the crunchy things [like potato chips!] no longer allowed in my diet. About half way through a cup of ice, the ice cream backache [or Spine Freeze, as I like to call it!] starts. It seems to be right in the middle about mid-back, right around where my bra strap is.

Wow! I just experienced the "ice cream backache" for the first time! Boy, does it hurt!! Right under my right shoulder blade near the middle of my back. I'm 5o and have never had it happened before. I googled the symptom and was happy to find this blog! I'd still like to know why, but it helps to know I'm not the only one.

I get the backache all along my middle spine, in the thoracic region. I assumed it was because I have thoracic arthritis and mild scoliosis - my back is already damaged so I feel pain more intensely there.
First time was drinking iced coffee on a very hot day in 2013.

This July 2014 I just started getting this severe sharp pain in mid back near my kidneys. It happened after I got a drive-thru slush drink on a very hot day. After a few big slurps through the straw as I was driving, I thought I was going to pass out from the intense back pain. I had to pull the car over and jump out and lean over and then it was gone. But I didn't know what caused it until I took another big ice cold drink and it started happening again...I use to get brain freezes and then I noticed they had stopped and I thought I was lucky...well I would rather have a brain freeze than a back freeze. I am sorry for all of you because I know how much it really hurts. Thanks for posting .

I just had a ice cream back ache and googled it and came across this blog. My husband watched on in horror like I was having a heart attack or something serious! Just like you said its the worst pain ever!!

I have had backaches while eating icecream every since I was in a car wreck 42 yes ago and broke my neck. As a nurse I believe it is because the normal track that nerves generally follow was disrupted. I drink something warm and it's gone. Don't need MD to diagnose. They wanted to do neck surgery when my shoulder froze up. Same thing. Inflammation travels to point of least resistance, in my case my spinal cord. Lolo

I have been asking those who happen to be with me when I eat something very cold too fast, "haven't you seen this before?" While I arc my back. "Don't you know anyone who feels a stabbing pain in between their shoulder blades?" I am 54 and have never had brain freeze. I am so pleased to have finally come across like-backed people!

Today I finished my grandson's icee and, on a hot day, slurped it through a straw a little too fast. I was driving and had to pull over because it hurt so much between my shoulder blades and on the roof of my mouth. It passed within a minute or two but it scared me enough to come home and look it up online. Glad to find out I wasn't having a heart attack! Really painful though.

I found this googling back freeze from ice cream because my family all thought I was weird! Yay, not alone in suffering this when eating ice cream or smoothies too fast. Used to be traditional brain freeze, but recently it moved to my back between my shoulder blades (I'm now over 40). Thanks for the links in your post as well!

I too have a back problem.. Fibromyalgia and am 37.. I do have migraines.. I read on another post that people who get migraines are "more likely" to get brain freeze and that only 30% of people get brain freeze.. the other evening at work I had the bartender make me a small strawberry smoothly.. I had never really thought about brain freeze much.. Not even enough to know it happens from usually drinking thru a straw because it has to do with the ice cold hitting the roof of your mouth.. When it did OH MY GOD!!!!! I had not only brain freeze but back freeze.. I remember I just stood there kinda arching my back and trying to look normal as a coworker asked me what was wrong...they don't care about me much there at work so no one payed much attention to me but one of my coworkers actually doesn't dislike me too much in front of my face and like I said asked what was wrong and I couldn't even talk.. I kinda paced shaking my head a bit.. Trying to hide the pain.. I didn't say a word durring.. didn't make a sound.. But had to literally gather myself back together out of this INTENSE pain shooting thru my mid spine.. I was sitting here after work a couple days later and thought about it remembering the pain like I can imagine a mother would always remember childbirth and I will always remember the pain of a migraine but THAT pain I almost arch my back and cringe at the thought of it.. (And remember my back is in horrible pain every day) but this was the very first time I've felt this pain and my google search led me here and this info helped a lot to read so I thought I'd share my story as your stories helped me so much!! I too am a weirdo back freezer. I have never experienced a brain freeze only a back freeze, right between the shoulder blades. No one has ever thought I was serious when they witness me flailing about in excruciating pain. In my 31 ywars i have never met anyone who has experienced one. Pretty cool to know I'm not the only one....why didn't I Google this a few years back?

Meet too got the same problem after eating ice cream, I ate icecream very quickly, may be this caused lower back pain in kidneys part, thought I was the only one

I have gotten a sharp pain between my shoulder blades when eating anything very cold my entire life. I finally decided to Google it today and came across this blog. I'm so glad there's others out there who have this problem! No one I know has ever felt it. I rarely get brain freeze, but every single time I eat ice cream or anything cold, after the first bite or sip, I get the backache and writher in pain for 30 seconds while it passes. It sometimes even happens if I drink a very cold can of LaCroix. So strange!

I experience brain-freeze in my neck/back of my head! I asked my professor who specializes in biology of the head and neck, and he'd never heard of this before. He said maybe it could be caused from stimulation to the Vagus nerve or Glossopharyngeal nerve. Or, constriction of the blood vessels in the back of my throat as a result of cold stimulation. It's a neat medical mystery.

mine started after a severe car accident. I helped a friend finish off a frozen drink the other night and I about passed out. It's happened before and I live in a lot of pain every day but this is enough to take anyone down

I too am happy to know I'm not alone with the spine freeze. My theory is that it may have to do with a narrowing of blood vessels in the brain stem or a nerve abnormality.

My back freeze rarely happens with ice cream, but slushies get me every time!! My pain is intense in my temples as well, but the neck and between the shoulder blades is the worst! I'm 30 and feel like I remember this happening mostly within the last 5 years. Never had any back or neck injuries.

I also know this is old I am 69 and only recently the last year maybe two I've had ice cream back right in the middle of my bra line at the back I have degenerative spinal disease and thought it was something to do with it but this has put my mind at rest because now I know that I'm not the only one thanks I'm from London England

Completely freaked my mom out today at Sonic with this (33 years old, and it was a first for her)... but mine hits in multiple places. Right eye, between my shoulder blades, lower right back, and the back of my throat. This time it was so bad that I started sweating and thought I was going to throw up. It's awful! But only with frozen drinks...ice cream doesn't bother me.

Im SO happy i came across this. I have a two year old and she's been complaining of her back hurting so much this summer. I realized it was only when were eating frozen fruits and drinking our smoothies. So interesting

I experience both brain and back. It's quite excruciating. Especially when I am drinking a frozen drink. I wonder if a medical condition can exacerbate the sensation. Honestly, its some of the worst pain I've ever experienced, fortunately it only last a few minutes.

I have always had back freeze and thought I was just weird. Just recently I found out my grandson also gets it and my husband has just experienced his first one. So happy to find this sight and found out there are many fellow sufferers. I've learned to just eat my ice cream slowly so that it is not so cold when I swallow it.
